Gar nicht. Das legt immer noch der Client (bzw. der Benutzer) fest, wie (oder mit welchem Programm) er eine Datei öffnet.

Du kannst mit PHP z.B. eine pdf-Datei so senden, dass sie auf keinen Fall im Browser angezeigt wird, sondern dass ein "Speichern unter..." Dialog kommt. Aber eine Anwendung vorschreiben kannst du nicht.
