Ist zwar etwas offtopic, aber ich stelle die Frage trotzdem.

Was ist das? Wozu braucht man das? Und die wichtigste Frage: Brauch ich das auch??? Noch nie gehört!

ich denke das dieses Formular wieder nur von speziellen IHK verlangt wird, wie z.B. IHK Emden.

Das ist einfach ein Formular, auf dem der Auszubildende und der Ausbilder mit Ihrer Unterschrift bestätigen, dass dieses Projekt von dem Auszubildenden durchgeführt wurde.

Ich habe mir das Formular von einem Lehrer, der selbst im Prüfungsausschuss sitzt, per E-Mail schicken lassen.
